What is Dental Insurance?

A health and wellness plan designed to pay a portion of dental costs associated with preventive, basic, and some major dental care, as well as orthodontia.

 

Plan Rules:

  • Eligibility: Eligible employees, as described on page 1 of this benefits guide, spouse, & dependent children (up to age 26)

  • Coverage through MetLife

  • In-Network provider Directory: CLICK HERE (Network: PDP PLUS)

  • Orthodontics available for children only <19 years old (subject to takeover provision)

  • No waiting periods or late entrant penalties

  • The chart below is a sample of covered services.  Please see the Plan Certificate available for full details
